Job Summary

This position is responsible for providing high-quality nursing care to residents in a skilled nursing setting. The successful candidate will be a licensed practical nurse (LPN) with experience in charge nurse roles and a passion for delivering exceptional patient care.

Key Responsibilities
  • Assist with formulating a plan of care for residents in the skilled nursing area.
  • Provide nursing care to residents according to physician orders and regulations.
  • Update plan of care as needed and document observations and care given to residents.
  • Use the electronic medical record (EMR) accurately and ensure timely follow-up of signs and symptoms of resident infections.
  • Direct and assist with resident, family, and significant other education.
  • Notifies family on condition changes and handles linens properly.
  • Use standard precautions by using personal protective equipment and observing transmission guidelines.
Support on the Unit

The skilled nursing unit is supported by a team of certified nursing assistants (CNAs) and psychiatric technicians. The charge nurse will work closely with the team to ensure seamless care delivery.

Charting System/Equipment

The skilled nursing unit uses the EPIC charting system, and the charge nurse will be responsible for ensuring accurate and timely documentation.

Physical/Mental Requirements

The physical demands of this role include standing, walking, and lifting, as well as frequent communication and observation. The charge nurse must be able to effectively communicate in English, both orally and in writing.

Requirements
  • Knowledge and familiarity with the therapeutic effects and side effects of all medications administered.
  • BLS certification from the American Heart Association.
  • Graduate of an accredited school of nursing.
  • Prior work experience in a like or related position may be helpful.
  • Licensure for the state of practice is required.
