Key Responsibilities- Provide comprehensive primary care, including health assessments, diagnosis, treatment, and management of acute and chronic conditions for patients of all ages.
- Conduct routine physical exams, order and interpret diagnostic tests, prescribe medications, and develop individualized treatment plans.
- Educate patients on health promotion, disease prevention, and management of chronic illnesses such as diabetes, hypertension, and asthma.
- Collaborate with a multidisciplinary team of physicians, nurses, social workers, and specialists to ensure holistic, integrated care for patients.
- Address urgent care needs, including minor injuries, infections, and other acute health issues.
- Engage in community outreach and health education initiatives to promote wellness and preventive care in the community.
- Maintain accurate and detailed patient records in compliance with healthcare regulations and clinic policies.
- Participate in quality improvement initiatives and continuing education to stay current on best practices in primary and preventive care.
- Current licensure as a Family Nurse Practitioner (FNP) in NY.
- Minimum of 1 year of experience in primary care, family medicine, or community health preferred.
- Strong clinical skills and ability to work independently in a community health setting.
- Excellent communication, interpersonal, and problem-solving skills.
- Commitment to providing culturally competent, compassionate care to underserved and diverse populations.
