Care Management Nurse Job Description

The Dimock Center is seeking a highly skilled and compassionate Care Management Nurse to join our team. As a key member of our care management program, you will play a vital role in providing intensive case management services to high-risk patients.

Key Responsibilities:
  • Screen and assess patients for common chronic diseases
  • Facilitate patient engagement in primary care and enrollment in the program
  • Conduct phone triage of patient calls and provide education on common chronic diseases and treatment options
  • Monitor patients for changes in clinical symptoms and treatment side effects or complications
  • Support medication adherence and coordinate referrals to indicated services
  • Act as liaison between patients, families, and healthcare teams to ensure effective communication and understanding of treatment plans
  • Collaborate with the quality improvement team to enhance and refine clinical workflows
Requirements:
  • License/Associate's Degree in Nursing or related field
  • Experience working with diverse populations (2-3 years)
  • Knowledge of database management, electronic health records, and population health interventions
  • Demonstrated ability to collaborate and communicate effectively in a team setting
Preferred Qualifications:
  • Master of Science in Nursing (MSN)
  • Systems thinking experience and/or training (e.g., Lean Six Sigma)
