Comprehensive Home Care Clinical Advisor

About the Role:
In this critical position, you will directly complete and oversee the development of patient treatment plans, perform ongoing evaluations of patient needs, and coordinate with a team of professionals to ensure optimal clinical outcomes. Your expertise will be instrumental in ensuring consistent care scheduling and dissemination of updated clinical information.
Essential Responsibilities:
- Perform initial and ongoing evaluations of patient needs within your scope of practice
- Complete Start of Care/Resumption of Care/Recertification/Discharge of OASIS visits and provide routine visits as needed
- Ensure patient care plans incorporate health maintenance, prevention, and safety education
- Coordinate available resources to manage care plans and achieve stated outcomes
- Periodically reassess or delegate patient needs reassessment and revise care plans as necessary
Requirements:
- Bachelor of Science degree in Nursing (BSN) preferred
- At least 2 years' clinical home care experience with minimum one year experience in a clinical management role preferred
- Current CPR certification
- Excellent verbal and written communication skills
- Excellent interpersonal skills
- Knowledge of state and federal home health agency regulations and compliance standards
- Ability to implement clinical policies and procedures
- Knowledge of PDGM clinical structure
