The Laurels of Goshen is seeking a highly skilled and compassionate Social Work Professional to join our team. As a key member of our healthcare staff, you will play a vital role in providing exceptional care and support to our residents.

Key Responsibilities:

  • Conduct thorough social services evaluations to identify the needs of our residents and develop personalized care plans.
  • Provide guidance and support to residents and their families on financial and legal matters, including assistance with applying for pensions and referrals to lawyers and funeral homes.
  • Collaborate with interdisciplinary teams to develop and implement discharge plans that ensure a smooth transition for residents to community-based care or other facilities.
  • Offer counseling services and psychiatric medication management as needed to support the mental and emotional well-being of our residents.
  • Ensure accurate and timely documentation of resident care plans in our electronic medical records system.

Requirements:

  • Current social work licensure in the state is preferred, but not required.
  • Bachelor's degree in social work, psychology, or a related human services field is required.
  • One year of social work experience in a skilled nursing facility setting is preferred, but not required.
