Location and Setting

The position is based in Brooksville, FL, at the Oak Hill Health and Rehabilitation Center. The center is a 109-bed facility that offers a range of services, including skilled nursing, assisted living, and rehabilitation.

Job Responsibilities
  • Provide occupational therapy services to patients in a skilled nursing and assisted living setting.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and optimal patient outcomes.
  • Conduct assessments, evaluations, and progress monitoring to inform treatment decisions.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
Requirements and Preferences
  • Master's degree in Occupational Therapy from an accredited program.
  • Current licensure as an Occupational Therapist in the state of Florida.
  • At least 1 year of experience in a skilled nursing or assisted living setting.
  • Strong clinical skills and ability to work independently.
  • Excellent communication and interpersonal skills.
