Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in Oakton, Virginia.

We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes. As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized plans of care.

• Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
• Evaluate patient needs and develop individualized plans of care to improve functional abilities and promote independence.
• Conduct thorough assessments and document patient progress, ensuring accurate and timely reporting to the attending physician and other healthcare professionals.
• Collaborate with patients, families, and caregivers to educate them on proper use of equipment, techniques, and strategies to promote optimal outcomes.
• Communicate effectively with the healthcare team to ensure seamless care coordination and patient satisfaction.

• Current licensure as an Occupational Therapist in the state of Virginia.
• Minimum one year of experience as an Occupational Therapist in an acute care setting, with preference given to those with home care experience.
• Valid driver's license and auto insurance.
• Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
