About the Role

As a Geriatric Care Specialist, you will provide proactive and evidence-based therapy to older adults, delivering geriatric house calls and developing personalized care plans to help them achieve their goals.

Responsibilities
  • Provide geriatric house calls to older adults
  • Deliver evidence-based therapy to older adults
  • Develop and implement personalized care plans
  • Collaborate with healthcare teams to ensure comprehensive care

Requirements

To be considered for this role, you must have:

  • A valid Physical Therapy Assistant license in the state(s) of practice
  • A degree from an accredited occupational therapy assistant program
  • Basic computer literacy skills
  • Current CPR certification
