Nurse Care Specialist

We are seeking a highly skilled Registered Nurse to join our team at Sonder Healthcare. As a key member of our home health and hospice care team, you will be responsible for providing high-quality patient care and coordinating services to meet the unique needs of individuals and families in their homes and communities.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine home care needs and develop individualized care plans.
- Provide direct patient care, including administering medications and treatments as prescribed by physicians, and counseling patients and families on health and wellness.
- Coordinate with community health-related persons to ensure seamless care transitions and effective communication.
- Participate in on-call duties and ensure that arrangements for equipment and other necessary items and services are available.
- Supervise and evaluate home health aide care provided to patients.
- Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting.
- Current license in state and CPR certification.
- 1 year of home health care experience preferred, but not required.
- Excellent observation, verbal and written communication, problem-solving, and basic math skills.
- Ability to stand or walk for prolonged periods and lift, position, or transfer patients, supplies, and equipment.
- Visual acuity and hearing to perform required nursing skills.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
