Job Summary

We are seeking a skilled and compassionate Pulmonary Nurse Navigator to join our team at Hackensack Meridian Health. As a key member of our healthcare team, you will play a vital role in coordinating and facilitating the care of patients with primary pulmonary care needs.

Key Responsibilities
  • Collaborate with physicians, nursing staff, and interdisciplinary teams to assess, plan, and implement care for selected patients and their families.
  • Screen patients for potential eligibility to the pulmonary transitions of care program and enroll eligible patients.
  • Conduct patient/family interviews to assess needs and develop individualized needs assessments.
  • Develop person-centered transitional care plans in collaboration with patients, families, caregivers, and healthcare teams.
  • Provide patient and family education regarding diagnosis, treatment, and self-care management.
  • Facilitate communication and coordination between healthcare team members and involve patients/families in decision-making processes.
  • Ensure timely and safe transitions of patients to the next appropriate level of care.
  • Maintain current and up-to-date information on community resources and refer patients to those resources that will enhance their lives and clinical outcomes.
Requirements
  • Bachelor's degree in nursing from an NLN/AACN accredited program.
  • Minimum of 2 years experience as a registered nurse with care of pulmonary patients or chronic/acute on chronic disease management.
  • Strong organization and problem-solving skills.
  • Excellent critical thinking skills.
  • Ability to work in a fast-paced team environment and prioritize/multitask.
  • Ability to make sound, independent clinical judgments and act professionally under pressure.
  • Demonstrate ability to provide age-appropriate skills, cultural competency, and customer service skills and health literacy.
  • Excellent written and verbal communication skills.
  • Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
Preferred Qualifications
  • Master's degree in nursing or related field.
  • Minimum of 2 or more years of experience as a registered nurse in leadership, education, case/care management, or social work.
  • Project management skills.
