Program Coordinator, Wickliffe Family Resource Center

Signature Health is seeking a highly motivated and detail-oriented Program Coordinator to join our team at the Wickliffe Family Resource Center. As a Program Coordinator, you will be responsible for developing, coordinating, and promoting activities at the center, collaborating with community partners and residents to connect individuals and families with appropriate services and resources.
Key Responsibilities- Coordinate the provision of services, supports, education, and training through providers chosen by the person and identified on their place of care.
- Serve as the primary point of contact for intra and inter agency/service coordination follow-through for all programs at the FRC.
- Coordinate day-to-day activities at the FRC, ensuring access for programs and providers to use the center during and after business hours.
- Coordinate communication of programs and services to the Wickliffe community and surrounding areas, including but not limited to students, families, and other community residents and agencies.
- Manage social media presence for the FRC.
- Assist staff to facilitate access to student information as appropriate for service providers working out of the FRC.
- Ensure all evidence-based services, supports, education, and training are implemented consistently as determined by ongoing needs assessment, available funding, and community interest and utilization.
- Utilize formal and informal mechanisms to periodically assess satisfaction of results from provided services, supports, education, and training.
- Monitor, report, and track data related to the volume and effectiveness of services and programs offered at the FRC.
- Provide, arrange for, and review documentation of services, supports, education, and training and their ability to address social determinants of health.
- Represent the FRC, Wickliffe Schools, and Signature Health in the community, attending meetings, marketing/PR events, and other forums to assist in expansion and development of FRC/Signature Health programs.
- Participate as needed in fundraising/grant writing activities.
- Maintain strong relationships with Wickliffe community partners and clients at the FRC, maintaining confidentiality of persons utilizing services.
- Perform other duties consistent with the mission/vision of the FRC.
- Bachelor's degree in Public Health, Health Education, Health and Human Services, or Marketing/Business Administration or a related field preferred.
- Experience in working in a community healthcare setting, outreach program management, public relations, event management, marketing, and communications or a related role required.
- Ability to handle sensitive information and contact with individuals, families, and legal guardians.
- Excellent verbal, written, and interpersonal communication skills.
- Must possess a valid unencumbered Ohio driver's license.
General office setting at the Wickliffe Family Resource Center. Work performed in the community may subject some positions to adverse weather conditions and challenging interaction with off-site locations and community members. Some travel may be required; reliable transportation, valid driver's license, and current auto insurance and the ability to maintain insurability. Work involves potential exposure to patients experiencing agitation, which may increase risk for disruptive or aggressive behavior. Flexibility to work evenings and extended hours is required. Requires periods of sitting, standing, telephone, and computer work. Hearing: adequate to hear clients or patients in person, over the telephone, or through telehealth technology. Speaking: adequate to speak to clients or patients in person, over the telephone, or through telehealth technology. Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens. Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted. Possible exposure to blood-borne pathogens while performing job duties. Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting, and sitting, with some lifting, pushing, and pulling exerted regularly throughout a regular workday. Sufficient dexterity to operate a PC and other office equipment.
