Qualifications

  • Supervised Post-Certification experience in a clinical setting (1+ years)
  • Minimum of 1 year experience in any of the following:

Overview

The Speech Language Pathologist will deliver speech therapy services to Agency patients as prescribed by the attending physician. This role includes providing consultation to Agency staff and the patients' physician regarding speech therapy as needed.

The responsibilities encompass assessing, planning, and executing rehabilitation services for speech, language, and/or swallowing disorders. Additionally, the Speech Language Pathologist will evaluate speech and language capabilities through diagnostic assessments.

The Speech Language Pathologist will also be accountable for documenting and reporting the client's progress in treatment and any changes in the patient's condition to the physician. Furthermore, they will instruct family members and healthcare team members on methods to assist the patient in enhancing communication skills, while maintaining comprehensive clinical and progress notes.

Key Responsibilities
  • Assess, plan, and implement rehabilitation services for speech, language, and/or swallowing disorders.
  • Conduct evaluations of speech and language abilities through diagnostic testing.
  • Establish goals and estimate prognosis for recovery.
  • Document and report the client's progress in treatment and any changes in the patient's condition to the physician.
  • Instruct family members and healthcare team members on methods to assist the patient in improving communication skills.
  • Re-evaluate patient progress and the established rehabilitation program as necessary.
  • Prepare periodic reports as required by the Agency for the physician and third-party payers regarding the care provided.
  • Assist in making appropriate referrals to external audiology speech clinics or other community services as indicated.
  • Maintain adequate clinical and progress notes and submit all required documentation promptly according to Agency policy.
  • Provide liaison contact and follow-up with other community agencies.
  • Participate in in-service training, case conferences, and staff meetings as required.
  • Maintain and enhance professional competency through in-service attendance and similar professional resources.
  • Perform all other duties as assigned by the supervisor.
Schedule and Shift Details

The employee will be required to drive daily for meetings and/or visits to residential homes. The work environment will vary based on the assigned caseload.

Travel

The employee will travel by automobile and may be exposed to changing weather conditions.
