Job Summary

The Clinical Coordinator I provides individual, group, and family therapies for individuals residing in the Residential Treatment Center. This role serves as a member of the resident's treatment team, coordinating various components of treatment both internally and externally with other providers.

Key Responsibilities
  • Assist in the intake process by coordinating a preplacement visit and evaluation for appropriateness of individual referred for admission to Adelbrook.
  • Help oversee the coordination of intakes, ensuring completion of all admission paperwork, setting up resident's files for admissions, and distributing materials to appropriate departments.
  • Responsible for completing the psychosocial assessment, as well as developing and implementing a comprehensive treatment plan for each assigned individual on assigned caseload.
  • Coordinates implementation of treatment plan(s) with other team members through weekly treatment team meetings.
  • Coordinates the admission and transitional process for residents/individuals, including maintaining communication with Beacon Health, DCF, School Districts, and DDS Regions.
  • Provides individual, group, and family therapies for all individuals residing in the residential treatment center assigned to one's caseload, as well as participates in psychiatric rounds for each individual.
  • Works to identify professional development needs of staff and appropriate training. Collaborates with the administrative team to address these needs.
  • Collaborates with the Director of Residential and Supervisor of Clinical Services to ensure that all staff have competencies to provide services that meet individual/residents' needs.
  • Communicates and collaborates with clinical services staff, behavioral service staff, contracted professional staff, and administrative team members to develop and maintain a professional work environment that beneficially meets the needs of the residents/individuals.
  • Works collaboratively with all professional disciplines to provide the most comprehensive and therapeutic services to each individual.
  • Participates in clinical staff meetings, supervision, case presentations, and all other required clinical meetings to enhance the individuals' treatment goals.
  • Serves as the primary liaison between Adelbrook, the family/guardian, educational placement, funding source, and all other appointed services, for the individual.
  • Ensures visitation for each assigned individual with family/guardian is supported clinically and communicated to all parties as needed.
  • Maintains and updates records, in a timely fashion. Completes all treatment summaries, progress notes, initial assessments, MTTPRs, and discharge summaries in collaboration with all internal professional disciplines, the Supervisor of Clinical Services, and the Director of Residential as prescribed by PNMI and ethical standards.
  • Oversees the maintenance of files (EHR and hard files) to ensure that they are maintained in a confidential and orderly manner. This includes preparation for PNMI, DCF, and COA licensing visits.
  • Ensures that all DCF contract licensing, PNMI, and COA best practices are followed throughout all programs.
  • Performs on-call every other weekend, Friday evening through Monday morning, or as assigned by the Director of Residential and/or the Supervisor of Clinical Services. During on-call hours, s/he must be available to handle emergency situations that occur outside of normal business hours.
  • Coordinates discharge planning in collaboration with all applicable professionals, including external agencies.
  • Maintains professional competency by participating in workshops, seminars, and all other requirements mandated by one's licensure and/or discipline.
  • Displays exemplary role modeling on the job and when in contact with the individuals being served.
  • Perform all other duties as assigned by the Director of Residential and/or the Supervisor of Clinical Services.
Requirements

A Master's degree in Marriage and Family Therapy, Social Work, or related field from an accredited institution is required; Clinical license preferred. All MSW's and MFT's must possess their LMSW and LMFT Associate to be eligible for hire. S/he must have a demonstrated ability to collaborate with other agency employees, community service organizations, the Department of Children and Families, and the Department of Developmental Services. The ability to work with persons of diverse ethnic, economic, educational, and religious backgrounds is required, as are excellent verbal and written communication skills.
