Weekend Hospice Nurse

The Visiting Nurse Association of the Treasure Coast is seeking a skilled RN to provide weekend nursing care to patients in their homes, hospitals, and nursing facilities. As a member of our interdisciplinary team, you will have the opportunity to work with patients and their families to provide compassionate, high-quality care at the end of life.
Responsibilities- Assess patient needs and provide supportive care in accordance with the attending physician's orders and plan of care.
- Communicate with attending physicians, patient care managers, and other healthcare professionals to coordinate optimal care and use of resources.
- Document patient information in our electronic medical record system, ensuring accuracy and compliance with regulatory requirements.
- Respond to medical or emotional crises, and provide direct care as needed, including support at the time of death.
- Evaluate patient condition and eligibility for hospice care, and complete the admission process as needed.
- Educate primary caregivers, volunteers, and employed caregivers on patient care and provide support as indicated.
- Participate in regular meetings with hospice nursing staff to review problems, share professional support, and exchange feedback.
- Participate in VNA Hospice orientation and in-service training programs to enhance professional growth and understanding of hospice care.
- Provide on-call coverage during designated holidays.
A graduate of an accredited registered nurse program and a valid Florida RN license are required. Previous hospice experience is also necessary.
