Job Summary

We are looking for a highly motivated and experienced Occupational Therapist to provide high-quality patient care in a Skilled Nursing Facility (SNF) setting. The ideal candidate will have excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient outcomes.

Responsibilities
  • Assess patient needs and develop individualized treatment plans to meet their unique goals and objectives.
  • Provide skilled occupational therapy services, including evaluations, interventions, and progress monitoring.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care and achieve optimal outcomes.
  • Document patient progress and maintain accurate records to ensure continuity of care.
  • Participate in quality improvement initiatives and contribute to the development of best practices in occupational therapy.
Requirements
  • Current state license as an Occupational Therapist.
  • Graduation from an accredited school of occupational therapy.
  • Current CPR certification.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills.
