Job Title: Physical Therapist Assistant

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapist Assistant to join our team of skilled home health care providers. As a Physical Therapist Assistant, you will work under the direction of a Physical Therapist to provide client services that have been delegated by the PT.

Responsibilities:
  • Prepare clinical and daily progress notes, reviewed by the PT and submitted according to company policy.
  • Perform routine strengthening and therapeutic exercises and assist patients with the use of self-help devices.
  • Instruct clients and family members on exercise programs and assist in teaching exercise techniques.
  • Identify potential medication issues and report to the PT.
  • Record activities and findings in the clinical record and report to the PT and physician any adverse reactions to treatment.
  • Consult with the PT regarding the outcome of home visits and report physical findings.
  • Instruct other health team personnel on physical therapy techniques.
  • Write daily progress notes and submit according to company policy.
  • Participate in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary.
Qualifications:
  • Graduate of an accredited Physical Therapy Assistant program or equivalent education.
  • Licensed to practice as a PTA in the State of Florida.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Minimum of one year's experience in home healthcare preferred.
  • Electronic medical record experience is preferred but not essential.
