## Job Title: Home Health LPN School Chaperone

### Job Summary

We are seeking a compassionate and skilled LPN to join our team as a Home Health Nurse and School Chaperone. As a Home Health Nurse, you will provide high-quality patient care to children in their homes, while also accompanying them to school. This is a rewarding opportunity to make a positive impact on the lives of our patients and their families.

### Responsibilities

* Provide quality patient care to children in their homes, including administering medications, wound care, and other medical treatments
* Accompany patients to school and provide support as needed
* Collaborate with patients' care teams, including physicians, supervisors, and other medical providers
* Participate in patient and family education related to disease processes and care needs
* Record and report changes in patients' conditions
* Use effective relationship management techniques to communicate with patients' families and friends

### Qualifications

* Current RN licensure with the State Board of Nursing in the State of Florida
* 1-year experience as a Registered Nurse (RN) in the last 3 years in a supervised setting
* Current CPR in compliance with the American Heart Association standards
* Negative TB/PPD or Chest X-Ray within the last 12 months
* Successful completion of the skills assessment specific to the areas of experience
