Inpatient Pharmacist

As a skilled Inpatient Pharmacist at Kaiser Permanente, you will play a vital role in ensuring the optimal therapeutic results and cost-effective prescribing for our patients. Under indirect supervision, you will consult with patients and medical personnel regarding medication therapy, dispense compounds, procure, store, and distribute pharmacy products. Your expertise will be invaluable in providing medical personnel and patients with medication and information, as well as product identification.
Key Responsibilities:
- Monitor and/or manage the medication therapy of specific hospitalized or ambulatory patients to ensure optimum therapeutic results and cost-effective prescribing.
- Review laboratory test results and patients' progress to detect adverse drug reactions, and confer with medical personnel in laboratory test interpretation, care, and treatment of patients.
- Document patient information in the electronic medical record and other systems as appropriate, and advise patients on medication use, storage requirements, importance of compliance, precautions, and warnings.
- Provide medical personnel with medication information, drug therapy options, and pharmacokinetic dosing advice, and order and adjust dosages, order laboratory tests, and administer medication under protocol according to established policies and procedures.
- Dispense, compound, procure, store, and distribute pharmacy products, monitor the effectiveness and accuracy of medication distribution, and review physician orders and enter orders in the patients' electronic medical record.
- Verify accuracy of transcription or data entry in the medication administration record or electronic medical record, and verify accuracy of medication preparation and dispensing by pharmacy personnel.
- Participate in assigned education and training programs, and provide inservices and educational programs for pharmacy, other medical personnel, and patients as necessary.
- Perform other duties as required, and provide direction to Pharmacist Residents, Pharmacist Interns, Pharmacy Technicians, and other non-licensed personnel.
Basic Qualifications:
- B.S. degree in Pharmacy or Pharm.D required.
- Pharmacist License (California) within 1 month of hire.
- Basic Life Support certification required.
- National Provider Identifier required at hire.
Additional Requirements:
- Comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, disease states, and related information for a medical management plan.
- Knowledge of aseptic technique, and the preparation of sterile IV products and chemotherapeutic agents.
- Knowledge of emergency medications and drug monitoring.
- Possess excellent verbal and written communication skills.
- Must be able to operate the pharmacy computer system(s) and other equipment.
- Must be able to generate prescription labels.
- MUST BE ABLE TO PASS BACKGROUND CHECK.
- Must be able to work in a Labor/Management Partnership environment.
