Job Responsibilities- Deliver post-acute care as an LPN
- Monitor residents' physical and mental well-being and track vital signs
- Perform intakes and document residents' health concerns and medical histories
- Review medical records and charts
- Administer and verify medications
- Maintain medical supplies and contribute to a safe environment
- Assist with IV setup, vaccinations, and phlebotomies
- Follow and adhere to care plans
- Help medical staff with orientation and training
- Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
- Completion of an approved LPN educational program
- High school graduation, GED, or equivalent
- At least 6 months of experience (LPN or similar role)
- Ability to lift a minimum of 50 lbs
