About the Role:
As a Nurse Case Manager at Research Medical Center, you will play a vital part in ensuring high-quality, patient-centered care. Your primary responsibility will be to manage rehabilitative care to ensure optimum outcomes for our patients.
Key Responsibilities:
- Coordinate and direct care specific to the needs of each patient
- Collaborate with the Rehabilitation Program Director and Facility Case Management Director regarding departmental functions
- Ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences
- Facilitate the involvement of the patient throughout the rehabilitation process
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments
- Communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver
