Responsibilities
The selected candidate will collaborate with a physician to provide comprehensive medical services to patients. This role includes:
- Conducting initial assessments, gathering patient information, and preparing charts for physician review.
- Developing individualized care plans in partnership with the physician.
Core Duties:
- In conjunction with a physician, expand individualized primary healthcare services by diagnosing and managing acute or chronic health conditions.
- Collect patient histories and perform assessments through observation, interviews, and examinations. Order and interpret diagnostic tests.
- Provide direct treatment and manage health conditions, referring patients to other healthcare providers or community resources as necessary.
- Educate and counsel patients and their families regarding preventive health measures, treatment options, and available community resources.
- Document healthcare delivery and nursing processes in compliance with specialty standards and network policies.

Requirements
Licensure/Certification:
- BLS Provider certification within 30 days of hire.
- One or more of the following credentials is required:
- Nurse Practitioner certification from ANCC, Indiana Board of Nursing, or NCC within 180 days of hire.
- Registered Nurse certification from ANCC or Indiana Board of Nursing prior to hire.
Education:
- Master's degree in Nursing is required.
