Home Health Physical Therapist Opportunity

We are seeking a skilled Physical Therapist to join our Home Health team in Hood River, OR and surrounding areas. As a Physical Therapist in this role, you will provide high-quality patient care and work collaboratively with our team to deliver exceptional patient outcomes.

Job Summary

This is a full-time opportunity for a Physical Therapist to work in a dynamic Home Health setting. The ideal candidate will have a strong passion for delivering patient-centered care and a commitment to excellence in their practice.

Responsibilities
  • Provide skilled physical therapy services to patients in their homes, including assessments, goal development, and interventions.
  • Collaborate with our interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical function and mobility, identifying areas for improvement and developing targeted interventions to address these needs.
  • Develop and implement individualized treatment plans that address patients' goals and priorities, and provide ongoing evaluation and modification of these plans as needed.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and optimal patient outcomes.
Requirements
  • 1 year of experience in skilled Home Health as a Physical Therapist.
  • Current licensure in WA and OR.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
  • Ability to adapt to changing priorities and deadlines.
