Job SummaryWe are seeking a Full-time weekend RN position to join our team. As a Home Health Weekend Registered Nurse, you will be a field employee who performs skilled nursing visits and coordinates client care. You will be the point of contact for all disciplines involved with providing care to patients, overseeing the frequency of visits, and consulting with physicians and office staff as needed.
Responsibilities- Perform skilled nursing visits and complete coordination of client care
- Point of contact for all disciplines involved with providing care to patients
- Oversee the frequency of visits for the episode
- Consult with physicians and office staff as needed
- Perform accurate OASIS collection and ensure medication profiles remain current
- Ensure lab values are reported to physicians in a timely manner
- Attend weekly case conference and monthly case manager meetings
- Registered Nurses (RNs) must be currently licensed in the state of employment
- A minimum of one year of clinical experience is preferred
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
