Advanced Geriatric Care Provider

We are seeking an experienced Advanced Geriatric Care Provider to join our team at TruHealth LLC.
About the RoleThis is a dynamic opportunity for a highly skilled and motivated individual to improve patient outcomes in long-term care settings. The ideal candidate will have a strong background in primary care or internal medicine, preferably with geriatrics experience.
You will be responsible for providing personalized, coordinated healthcare to patients with complex medical and mental health needs, working closely with our facility partners and health plan leadership to implement our national model of care.
Responsibilities- Assess patients' medical and mental health needs, developing comprehensive plans of care that proactively manage their conditions and address barriers to care
- Ensure compliance with local, state, and federal regulations related to clinical services provided
- Prescribe medications and order lab work, diagnostic procedures, and consultations as needed
- Monitor patients' response to treatment and modify plans accordingly
- Collaborate with RN case managers as part of an integrated care team
- FNP, AGNP, AHACNP, or PA license required
- Degree from an accredited APRN or PA program
- At least 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook, and Excel
- An estimated annual salary of $120,000 - $150,000, depending on experience
