We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at At-Heart Home Care & Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families in the comfort of their own homes.
Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
- Administer medications and treatments as prescribed by the physician, and provide education to patients and families on self-care and disease management.
- Document patient progress and maintain accurate clinical records to ensure compliance with regulatory requirements.
- Participate in interdisciplinary team meetings to discuss patient care and address any concerns or issues.
- Graduate of an accredited nursing program with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is highly desirable.
- Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
