Psychiatric Social Worker Lead

Job Summary
Saint Vincent Hospital is seeking a skilled Psychiatric Social Worker Lead to join our team. As a key member of our healthcare community, you will be responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources.
Responsibilities
- Conduct complex psycho-social transition planning assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
- Assist with adoptions, abuse and neglect cases, including assessment, intervention, and referral as appropriate to local, state, and/or federal agencies.
- Care coordinate, implement, or oversee the implementation of transition plans.
- Lead and/or facilitate multi-disciplinary patient care conferences, including Complex Case Review.
- Making appropriate referrals to other departments, communicating with patients and families about the plan of care, collaborating with physicians, office staff, and ancillary departments.
- Ensure patient education is completed to support post-acute needs, timely complete and concise documentation in the Case Management system, maintenance of accurate patient demographic and insurance information.
- Precept new staff members and act as a resource to all staff, facilitate TEMPO as needed, participate in department quality improvement initiatives.
Requirements
- Master's of Social Work degree required.
- 2 years of acute hospital experience preferred.
- Accredited Case Manager (ACM) certification preferred.
Benefits
The successful candidate will enjoy a competitive salary of $$76,000 - $100,000 per year, based on experience, plus comprehensive benefits, including medical, dental, and vision coverage, 401(k) retirement plan, and paid time off.
