Key Responsibilities- Conduct in-home assessments to evaluate patients' medical history, review systems, and perform comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as needed.
- Identify diagnoses and develop care management plans to address patients' medical needs.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Intervene in urgent and emergent situations, providing timely and effective support.
- Educate patients on disease processes, medication management, and compliance.
- Active NP or PA license, or ability to obtain by start date.
- Active national certification in NP or PA, or ability to obtain by start date.
- Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting/standing/kneeling.
- Reliable transportation for home visits.
- 1+ years of clinical experience in a related setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, up to 5% of the time.
