Key Responsibilities
  • Conduct in-home assessments to evaluate patients' medical history, review systems, and perform comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as needed.
  • Identify diagnoses and develop care management plans to address patients' medical needs.
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination.
  • Intervene in urgent and emergent situations, providing timely and effective support.
  • Educate patients on disease processes, medication management, and compliance.
Requirements
  • Active NP or PA license, or ability to obtain by start date.
  • Active national certification in NP or PA, or ability to obtain by start date.
  • Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting/standing/kneeling.
  • Reliable transportation for home visits.
Preferred Qualifications
  • 1+ years of clinical experience in a related setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Ability to travel throughout the state, up to 5% of the time.
