Job Summary

We're seeking a highly skilled Nurse Practitioner to join our clinical operations team at ChenMed. As a key member of our team, you will provide direct patient care in our medical centers, nursing homes, skilled nursing facilities, and home settings.

Responsibilities
  • Conduct geriatric assessments, medical histories, physical exams, and develop nursing plans of care
  • Provide health education, physician referrals, and case management referrals
  • Collaborate with other healthcare professionals to ensure high-quality patient care
  • Participate in risk and quality management programs, clinical meetings, and other meetings
  • Adhere to strict departmental goals, objectives, and policies
Requirements
  • ARNP or similar advanced degree in Nursing
  • Minimum of 1 year of clinical experience (strong new graduates considered)
  • Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC)
  • Basic computer skills and knowledge of Microsoft Office Suite
