Silver Birch Living is seeking a dedicated and compassionate Wellness Nurse to join our Clinical Team. As a Wellness Nurse, you will play a vital role in providing exceptional care and service to our residents.

Responsibilities:

  • Manage medication orders and documentation in compliance with regulations.
  • Follow up with service providers to ensure resident needs are met.
  • Administer medications and treatments as ordered and document.
  • Initiate and document care delivered and care coordination, including completing admissions, assessments, transfers, and discharges.
  • Support and educate residents and families on proper healthcare directed by the resident care plan and promote compliance.
  • Provide exceptional customer service.
  • Oversight of QMA and CNA and support in resident care.
