About the Role

We are seeking a highly skilled Registered Nurse to join our team as a Diabetes Care and Education Specialist. This is a unique opportunity to make a meaningful difference in the lives of individuals affected by diabetes and cardiometabolic conditions.

Key Responsibilities

  • Deliver comprehensive and seamless services that integrate clinical and self-management aspects of diabetes and cardiometabolic care in all settings.
  • Collaborate with the interprofessional team to provide person-centered care and education conducive to behavior change and improved quality of life.
  • Advocate for and communicate about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardiometabolic conditions.
  • Apply business principles, systems practice, and population health management to support achievement of the Quintuple Aim.
  • Contribute to the achievement of established Diabetes Self-Management Education department goals and objectives.
  • Provide quality diabetes self-management education in individual and group settings based on assessed needs.
  • Collaborate with other members of the diabetes care team in developing person-centered diabetes plans.

Requirements

  • Associate, Bachelor's, Master's or Doctorate in Nursing.
  • Minimum of 1 year of clinical experience required.
  • Experience with diabetic patient population preferred.
  • Licensed to practice as an Registered Nurse in South Carolina.
  • Job related certification from one of the following: Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM).
