About the Role

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Hackensack Meridian Health. As a key member of our multidisciplinary clinical treatment team, you will play a vital role in providing comprehensive care to our patients.

Responsibilities
  • Conduct thorough psychosocial assessments to identify patient needs and develop effective treatment plans.
  • Provide evidence-based behavioral health treatment and education to patients, including individual and group therapy sessions.
  • Collaborate with patients, families, and other healthcare professionals to develop comprehensive discharge plans.
  • Assess and address Social Determinants of Health (SDOH) to ensure patients receive the best possible care.
  • Supervise social work/counseling students and provide guidance to other team members as needed.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • Minimum of 2 years of post-licensure work experience in a clinical setting.
  • Ability to work as part of a team and build collaborative relationships.
  • Excellent communication and interpersonal skills.
  • Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
Preferred Qualifications
  • Ability to provide guidance and mentorship to other team members.
