At Aveanna Healthcare, we are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN Part Time in Walworth County. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.
Job Summary:The Registered Nurse – Home Health is responsible for delivering skilled nursing care in accordance with the developed care plan and physicians' orders. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:- Develop and implement individualized care plans through physician orders, client input, and nursing assessment.
- Educate clients and their family members on specific needs and provide support.
- Properly orient and train primary caregivers to ensure optimal functioning levels.
- Coordinate continuum of hands-on client care through documentation and timely communication with physicians and caregivers.
- Provide care in patients' homes using various skills, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
- An active RN License in the state of application.
- Valid CPR certification.
- Medicare Skilled Nursing experience preferred.
- Basic understanding of Oasis.
- 1-year RN experience in a healthcare setting.
