We are seeking a skilled Nurse Care Specialist to join our team at Wellpath. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and support.
Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients' physical, psychological, and social needs, and develop individualized care plans to address these needs.
- Implementation and Evaluation: Implement care plans in collaboration with healthcare practitioners, and evaluate the effectiveness of care provided.
- Communication and Collaboration: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and delivery.
- Medication Administration: Administer medications as prescribed by healthcare practitioners, and monitor patients for any adverse reactions.
- Documentation and Reporting: Accurately document patient interactions, care provided, and any changes in patient status.
- Education: Graduation from an accredited School of Nursing.
- Experience: One (1) or more years of clinic experience preferred.
- Licenses/Certifications: Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN), and current CPR certification.
