We are seeking a skilled and compassionate Home Health Registered Nurse (RN) to join our team at Providence Elderplace in Gresham, Portland.
As a Home Health RN, you will be responsible for providing professional nursing care to patients in their homes, focusing on restoring their independence and promoting optimal health.
This per diem/on call position involves working day shifts, providing comprehensive care to patients with various medical conditions. You will assess, plan, and implement personalized care plans that meet the unique needs of each patient.
The Community Care RN role requires a deep understanding of assessing, planning, ordering, giving, delegating, teaching, and supervising professional nursing care delivery. You must apply knowledge of general nursing principles to promote patients' optimal health and independence.
Providence PACE is a Program of All-Inclusive Care for the Elderly that aims to keep older adults healthy and active in their communities through clinics, home visits, and other services. This is an excellent opportunity to make a meaningful impact on the lives of elderly individuals and their families.
To succeed in this role, you should have a strong foundation in nursing principles, excellent communication skills, and the ability to work collaboratively as part of a multidisciplinary team.
Here are some key qualifications and responsibilities:
- Oregon Registered Nurse License upon hire
- National Provider BLS - American Heart Association within 30 days of hire
