Job Summary

We are seeking a compassionate and detail-oriented Home Health Aide to join our team at Sunrise Point Health Care. As a Home Health Aide, you will provide personalized care to patients in their homes, following a one-on-one care plan. Your role will involve assisting patients with daily activities, maintaining their records, and building relationships with them to promote their mental and physical well-being.

Responsibilities
  • Assist patients with daily activities such as bathing, dressing, and grooming
  • Provide personal care services such as changing bed linens, doing laundry, and cleaning the home
  • Maintain accurate records of patient care, condition, progress, and problems
  • Build relationships with patients through conversation and activities to promote their mental health and alertness
  • Administer medication reminders and engage patients in exercises or other activities
Qualifications
  • Graduation from an accredited Home Health Aide program
  • High School Diploma or equivalent
  • One year of prior professional experience
  • Valid driver's license
  • CPR certification preferred
  • Ability to pass a background check and drug screen
