We are seeking an exceptional Advanced Practice Healthcare Professional to join our team at IHC Health Services, Inc. As a key member of our healthcare team, you will provide advanced level patient care and services in a fast-paced clinical environment.

**Key Responsibilities:

  • Patient Care Coordination: Assess patient needs, develop individualized care plans, and coordinate services to achieve optimal patient outcomes.
  • Clinical Education and Training: Provide education and training to patients, families, and other staff members on various aspects of healthcare, including disease management and prevention.
  • Clinical Collaboration: Work effectively with patients, families, and other healthcare professionals to ensure seamless care coordination and exchange of critical information.

**Requirements and Qualifications:

  • Licensure and Certification: Hold a current state license as an APRN or PA and certification as a Nurse Practitioner.
  • Education and Experience: Possess a Master's degree from an accredited institution and relevant experience in family medicine.
  • Additional Certifications: Eligible for State Controlled Substance License II through V and Federal DEA Certificate II through V.

**Estimated Annual Salary:** $130,000 - $160,000 based on experience and qualifications.
