About the RoleWe are seeking an experienced Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for developing personalized care plans for each resident based on their unique needs and preferences. You will work closely with our interdisciplinary team to ensure that our residents receive the highest level of care and support.
Your primary responsibilities will include:
- Creating comprehensive care plans that meet the individual needs of each resident
- Collaborating with our healthcare team to provide high-quality care and services
- Monitoring resident progress and making adjustments to care plans as needed
- Communicating effectively with residents, families, and healthcare providers
We offer a competitive salary range of $ 85,000 - 115,000 per year, depending on experience, as well as a comprehensive benefits package, including:

To be successful in this role, you will need to have:
