Job Title: Licensed Practical Nurse (LPN)

At Third Street Family Health Services, we are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. Job Summary:

The Licensed Practical Nurse (LPN) will work under the supervision of registered nurses and physicians, assisting in the coordination and delivery of patient care services. Your primary focus will be on ensuring patient comfort, safety, and well-being while promoting efficient clinic operations.

Key Responsibilities:
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards in accordance with the Ohio Nursing Practice ethical and professional guidelines.
  • Accurately document pertinent observations and interventions in the Electronic Medical Record to aid in the development of the patient-centered care plan and the delivery of quality and equitable healthcare.
  • Carry out physician orders timely and accurately.
  • Maintenance of quality control standards, logs, and point-of-care testing.
  • Attends required Clinical and Health Center in-service programs.
  • Demonstrates responsibility and accountability for professional practice, continued growth, and self-evaluation.
  • Complies with the Third Street Family Health Services infection control policies and procedures.
  • Understands and demonstrates compliance with Health Services and Resources Administration (HRSA) and Third Street Family Health requirements.
  • Maintains appropriate licensure and participates in continuing education activities to exemplify the Licensed Practical Nurse.
  • Ensure and uphold the confidentiality requirements of all patient records, and manage all daily tasks and activities consistent with HIPAA, state and federal laws and regulations, as well as the clinic's policies and regulations regarding confidentiality and security.
  • Provide culturally competent quality patient-centered healthcare in partnership with the communities we serve.
  • Provides clinical and administrative support and functions as a resource and mentor for new staff and students assigned to their work site.
  • Assists the Clinical Nursing Manager in ensuring that nursing staff follow the Nursing Practice Act in their delivery of comprehensive patient care with maintenance of strict confidentiality.
Requirements:
  • Valid driver's license and reliable transportation
  • BCI/FBI Fingerprint (If applicable)
  • Graduate of an accredited school of Nursing.
  • Licensure: Ohio LPN Nursing License
  • Active Basic Life Support (BLS) certification.
