Position Overview
The Healthcare Coordination Specialist, LVN plays a crucial role in fostering collaboration, communication, and care management among all members of the patient's Home Health interdisciplinary team. The primary duties of this position include assisting the interdisciplinary team by acquiring physician orders, relaying patient lab results to physicians and RN case managers, and organizing patient appointments.
This role involves close collaboration with nursing leadership to address and resolve any issues that may arise. The Healthcare Coordination Specialist serves as a representative of Home Health while interacting with patients, their families, physicians, and the broader community.
Key Responsibilities
- Perform all tasks efficiently and effectively, aligning with departmental goals to ensure optimal service delivery and productivity.
- Assume additional responsibilities as assigned by the Clinical Director.

Required Experience
Mandatory:
- Current California license as a Licensed Vocational Nurse.
- A minimum of 1 year of professional LVN experience.
- Current CPR certification.
- Valid California Driver's License and auto insurance.
Preferred:
- Experience in Home Health or Hospice settings.
- Familiarity with the insurance authorization process.
