Job Summary:
Fast Pace Health is seeking a skilled Advanced Primary Care Provider to join our telehealth team. This role will involve providing high-quality patient care through direct contact and mediated communication.
About the Role:
The Advanced Primary Care Provider will be responsible for treating patients of all ages, including children aged 3 years and up. Key responsibilities include conducting thorough physical examinations, performing chronic care management, and administering annual wellness visits.
Key Responsibilities:
- Conduct thorough physical examinations and assessments for patients of all ages
- Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma
- Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings
- Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs
- Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans
- Presume medications, recommend treatments, and provide written home-going instructions
- Refer patients to specialists or other healthcare resources as needed
- Educate patients and their families on health maintenance and preventive care
- Promote patient engagement, health literacy, and shared decision-making
- Participate in quality improvement initiatives and value-based care programs
Requirements:
To be successful in this role, you will need to have a strong foundation in primary care and experience working in a telehealth setting. You should be able to work independently and collaboratively as part of a team.
