Job Summary

We are seeking a skilled Patient Care Manager to join our team. As a key member of our hospice operations, you will be responsible for managing the day-to-day activities of our clinical sites, ensuring the delivery of exceptional patient care and services.

Key Responsibilities
  • Manage and oversee clinical and administrative functions at assigned hospice sites
  • Respond to phone referrals and inquiries in a timely and professional manner
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes
  • Collaborate with interdisciplinary teams to ensure seamless patient care and services
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required
  • Three years' experience in direct patient care in a clinical setting required
  • Minimum one year of management/supervisory experience required
  • Current license to practice nursing in the state of employment required
