### Job Title: Caregiver / Direct Support Professional

### Job Summary

We are seeking a compassionate and dedicated individual to join our team as a Caregiver / Direct Support Professional. As a member of our team, you will play a vital role in supporting individuals with disabilities to live independently in their own homes.

### Key Responsibilities

* Provide personal care and support to individuals with disabilities, including assistance with daily living activities such as bathing, dressing, and grooming
* Develop and implement individualized support plans to promote independence and self-advocacy
* Build strong relationships with individuals and their families to understand their unique needs and preferences
* Collaborate with other team members to ensure seamless support and care

### Requirements

* High school diploma or equivalent required; degree in a related field preferred
* Experience working with individuals with disabilities or in a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Valid driver's license and reliable transportation
