Job DescriptionThe Case Manager RN PRN is responsible for coordinating patient care and ensuring a smooth transition between hospital stays. This role requires strong clinical skills, effective communication, and attention to detail.
Responsibilities- Conduct Comprehensive Assessments: Perform thorough evaluations of patients' medical and psychosocial needs, identifying areas for improvement and developing strategies for optimal care.
- Collaborate with Multidisciplinary Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless communication and coordination of care.
- Develop Effective Discharge Plans: Create tailored plans that address patients' specific needs, ensuring a safe and successful transition from hospital to home or next level of care.
- Facilitate Patient Throughput: Streamline the care process, minimizing delays and maximizing efficiency while maintaining high-quality patient outcomes.
- Clinical Experience: 3+ years of hospital nursing experience required, with at least 2 years of case management experience.
- Education: Graduate of an accredited nursing school, Associate's Degree required, Bachelor's Degree preferred.
- Certification: Certification in case management preferred.
