Job SummaryWe are seeking a Licensed Clinical Social Worker (LCSW) to join our team in Minnesota. As a LCSW, you will work with patients across the lifespan in an outpatient community mental health center setting, providing diagnostic assessments, treatment recommendations, and individualized treatment plans.
Key Responsibilities- Perform initial mental health diagnostic assessments with treatment recommendations.
- Develop individualized, prescriptive treatment plans to fit the needs of each patient using appropriate diagnostic criteria.
- Develop a strong rapport with a diverse set of patients exclusively through the telehealth modality.
- Document concurrently all clinical findings and services provided in the electronic medical record as required by standards of care.
- Participate in treatment meetings, consultation, and collaboration with members of the clinical team.
- Be present and available on-screen for the duration of the contracted hours.
- Relevant and active LCSW license in the state of Minnesota.
- Bilingual in Spanish and/or Somali.
- Certified and/or experienced in common therapeutic modalities such as CBT, DBT, TFCBT, and Motivational Interviewing.
- Excellent written, verbal, and listening communication skills, positive relationship building skills, and critical analysis.
- Flexibility and ability to adjust to the needs of the clinic partners.
- Well-versed in using EHRs, confident in a virtual setting, and concurrent documentation.
- Experienced in caring for patients with Serious Mental Illness.
- Experienced in caring for patients with co-occurring substance use disorders.
- Previous telemedicine experience and skilled at using online tools and technology to deliver care and communicate with patients.
