Job Summary

University of Minnesota Physicians is seeking a highly skilled and compassionate Certified Nurse Practitioner or Physician Assistant to join our multidisciplinary team in delivering exceptional care to patients with heart failure. As a key member of our team, you will provide expert patient care, coordinate services, and contribute to improving patient satisfaction and quality of care.

Key Responsibilities
  • Deliver high-quality patient care and coordinate services for adults with heart failure, including heart transplantation patients and mechanical circulatory support therapies.
  • Perform independent follow-up of patients in the clinic setting and participate in multidisciplinary advanced heart failure rounds in the inpatient setting.
  • Assess and formulate plans to address complex, acute, and chronic health needs, including ordering appropriate tests, labs, and interpreting results.
  • Communicate effectively with healthcare personnel, patients, and family members, and arrange for hospital admission and discharge as needed.
  • Participate in heart failure, transplant, and MCS meetings, and demonstrate a strong understanding of patient HF home education needs.
Requirements
  • Heart failure experience is preferred, and certification as a Certified Heart Failure Nurse (CHFN) through the AAHFN or eligibility for certification is also preferred.
  • A graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program, with eligibility or certification through NCCPA, ANCC, or AANP.
  • Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or MN APRN license or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP's).
