About the Role

We are seeking a skilled and compassionate Home Health Care Specialist to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide Direct Patient Care: Assist patients with daily living activities, such as bathing, dressing, and grooming, under the supervision of a registered nurse.
  • Assist with Medication Management: Help patients with self-administered medications, including opening bottle caps, reading labels, and observing medication intake.
  • Monitor Patient Health: Take accurate temperature, pulse, and blood pressure measurements, and report any abnormal results to the supervisor.
  • Assist with Personal Hygiene: Help patients maintain good personal hygiene by performing or supervising bathing, skin care, and other activities.
  • Assist with Nutrition and Meal Preparation: Assist patients with feeding, communicate basic nutrition principles, and observe and record food and fluid intake when necessary.
  • Assist with Mobility and Transfers: Help patients with mobility, exercises, positioning, and transfers, as per the Plan of Care and CenterWell Home Health policy.
  • Document Patient Care: Accurately document patient interactions, care rendered, and any changes or concerns in the patient's condition.
  • Practice Infection Control: Maintain a clean, safe, and comfortable environment for patients, and adhere to acceptable infection control principles.
Requirements
  • Education and Certification: High school diploma or equivalent, and completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
  • Experience: At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
  • State Certification: Must meet applicable state certification requirements.
  • Transportation: A valid driver's license, auto insurance, and reliable transportation are required.
