Job Title: Home Health Occupational Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in helping our clients achieve their goals and maintain their independence in the comfort of their own homes.

Job Summary:

We are looking for a highly motivated and experienced Occupational Therapist to provide high-quality care to our clients. As a Home Health Occupational Therapist, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each client.

Responsibilities:
  • Conduct thorough assessments of clients' functional status and develop individualized care plans to achieve favorable outcomes.
  • Provide occupational therapy services to clients in their homes, including evaluations, interventions, and education.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document client progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as an Occupational Therapist in the state of Pennsylvania.
  • Minimum of one year of experience as an Occupational Therapist in a home health care setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality care.
