Hospice RN Case Manager

Spero Hospice is seeking a dedicated and compassionate RN Case Manager to join our team. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals in the final stages of life.
Key Responsibilities- Patient Assessment and Care Planning: Conduct comprehensive assessments of patients' medical conditions, symptoms, and pain management needs, and develop individualized care plans in collaboration with the interdisciplinary team and patients' families.
- Nursing Care and Treatment: Administer skilled nursing care to patients, including wound care, medication management, and symptom control, and monitor patients' vital signs, reactions to treatments, and overall well-being.
- Emotional and Spiritual Support: Provide emotional support and counseling to patients and their families during the end-of-life process, and respect and accommodate patients' spiritual beliefs and cultural practices.
- Coordination of Services: Facilitate regular interdisciplinary team meetings to discuss patient care plans and address any issues or concerns, and coordinate the services provided by other members of the hospice team.
- Patient and Family Education: Educate patients and their families about the hospice philosophy, the end-of-life process, and available resources, and provide guidance on pain management techniques and strategies for coping with emotional challenges.
- Documentation and Compliance: Maintain accurate and up-to-date patient records, documenting all care provided and communicating with patients and families, and ensure compliance with all relevant regulations, policies, and procedures related to hospice care.
- Bereavement Support: Provide support to the family after the patient's passing, assisting with bereavement counseling and resources.
- Active Registered Nurse (RN) license in the state of practice in New Brunswick, NJ, US.
- Minimum of 2-3 years of clinical nursing experience, with a preference for hospice, palliative care, or related fields.
- Deep understanding of hospice philosophy and principles of end-of-life care.
- Excellent communication and interpersonal skills.
- Demonstrates empathy, compassion, and sensitivity when supporting patients and families through grief and loss.
- Ability to work independently and collaboratively within a team, showcasing strong organizational and time-management abilities.
- Proficient in using electronic health records and other relevant healthcare software applications.
