We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Marjorie Doyle Rockwell Center in Cohoes, NY. As a LPN, you will provide high-quality nursing care to our residents and work collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Key Responsibilities- Provide direct care to residents, including administering medications, performing vital signs, and providing wound care.
- Oversee a team of resident assistants and provide guidance and support as needed.
- Develop and implement individualized care plans for residents, taking into account their unique needs and goals.
- Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
- Valid NYS LPN license
- 1+ year of experience as a LPN, with a willingness to train and develop new skills
- 1+ year working in memory care in a direct care position highly desired
- A competitive salary range of $24.00 - $30.10, based on experience, skills, and education
