Job Summary

The Registered Staff Nurse provides comprehensive nursing services to individuals, families, and groups. She/he is responsible for the overall coordination of the Plan of Care and is proficient in all components of the nursing process.

Key Responsibilities
  • Skilled Nursing Care: Provides skilled nursing care to patients in their homes, including admission and discharge, on a family-centered basis, while recording and maintaining complete patient records.
  • Care Coordination: Teaches, demonstrates, supervises, and evaluates care given by peers, home health aides, family members, and significant others.
  • Patient Assessment: Systematically assesses patients' physical, psychosocial, emotional, cultural, spiritual, and discharge needs, according to age and developmental level.
  • Plan of Care Development: Develops and implements a plan of care for each patient, utilizing effective oral and written communication skills.
  • Plan Evaluation: Evaluates the plan of care and reformulates goals as necessary.
  • Technical Skills: Demonstrates accurate technical skills and operational knowledge of patient care equipment and procedures, including venipuncture to administer IV medication, TPN, lipids, etc.
  • Critical Thinking: Recognizes unsafe and life-threatening situations and intervenes appropriately.
Qualifications
  • Graduate from an accredited School of Nursing required.
  • Connecticut State license required.
  • Two years of recent medical/acute care or critical care experience as a registered nurse, or an equivalent level of experience preferred.
  • Achieve Home Health Care specific competencies within the time frame determined by the department.
  • Valid Connecticut Driver's License and proof of current insurance coverage required.
